Oral health and the risk of squamous cell carcinoma of the head and neck (HNSCC)

Tobacco use, alcohol consumption and human papillomavirus (HPV) infections have been established as the primary causes of head and neck cancer (HNC), while a few studies have shown mixed evidence that HNC risk may be associated with poor oral health and alcohol-based mouthwash. This study utilized CHANCE data, a study of both patients with HNC (cases) and healthy individuals (controls) and revealed that gum disease, as measured by self-reported tooth mobility, was associated with a 33% increased HNC risk but mouthwash use was not. Routine dental visits were associated with a 30% reduced risk, which highlights the importance of oral disease prevention from a public health perspective. This study demonstrates that more oral health indicators are necessary for future epidemiologic studies investigating the potential role of oral health in head and neck cancer risk.

Factors that impact health-related quality of life over time for individuals with head and neck cancer

Health-related quality of life (HRQOL) is a measure that assesses an individual’s perception of their spiritual, functional, physical, emotional and social well-being. Individuals with head and neck cancer experience disease- and treatment-related reductions in HRQOL. Poorly managed HRQOL have been previously reported to be associated with decreased overall survival. This study utilized CHANCE data to follow head and neck cancer (HNC) survivors over 5 years and found a number of demographic, behavioral, and clinical factors that were associated with different levels of HRQOL. Many of the HRQOL initial negative impacts, however, improve over time. Older patients reported better physical, emotional, and functional wellbeing and fewer symptoms than younger patients, regardless of comorbidity status and treatment. Higher educational status was also associated with a better long-term HRQOL, which likely reflects that education (as an indicator of socioeconomic status) is associated with better access to care and support networks. Similarly, private insurance was associated with better physical and functional well-being and less symptoms compared with no or government insurance. Lack of insurance was associated with greater financial burden from cancer care and has been found to be associated with poorer HRQOL. This study also supported the well-known fact that current tobacco and alcohol use are risk factors for HNC and both appear to be associated with poorer HRQOL for survivors. For the HRQOL factors, health care provider awareness can help to identify those at risk and provide more care and surveillance to reduce the negative effects of the cancer and its treatment.

Travel time to provider is associated with advanced stage at diagnosis among low income HNSCC patients in North Carolina

Early diagnosis of HNC may improve quality of life and outcomes for patients. Access-to-healthcare services has been linked with late diagnosis of many cancers including HNC. Akey determinant of access may be the distance that a patient must travel to obtain a diagnosis. This distance may disproportionately affect HNC patients, who may lack resources for transportation. These disadvantaged patients may delay seeking health care possibly due to difficulty reaching farther-away providers. Using the CHANCE cohort, researchers found that low-income HNSCC patients with longer travel time to the provider were more likely to be diagnosed with advanced tumor stage than higher income patients. This relationship was independent of other contributing factors such as medical insurance and rural location. Findings suggest that low socioeconomic status and limited resources for transportation may lead to patients waiting until symptoms increase before seeking a diagnostic biopsy. Furthermore, patients with limited resources may lack access to screening for head and neck cancer through routine medical or dental visits. Future research investigating the factors behind the association between distance and stage at diagnosis could provide insight for future interventions such as community efforts for screening, care coordinators, andtelemedicine.
